Interventions:

Client contacts office with any need to change plan of care
Attendant follows the written plan of care
Follows the current ordered frequency and duration
Consumer is satisfied with care
Changes made to the Plan of Care

Reporting:

Documents visit accurately
Documents is presented to office in the required timeframe
Current written Plan of Care present in home

Infection Control:

Washes hands (correct technique) and or uses alcohol gel
Demonstration proper waste disposal
Use appropriate home infection control measures
Use of proper body mechanics:
