Understand that I must turn in my time sheet to the office in order for a timely payment. Time sheets with original signatures (client and provider) are due in the office on ____ of every month. If time sheets turned in after that date paycheck will be issued late. If I am resigning from the position time sheets must be turned in as soon as last day of services occurred and paycheck will be given on the next payday. I understand that I have up to 45 days to turn in my time sheet to the office in order to receive a payment. Payment on late time sheets will be given on the next payday.
